My standard rate is $130 per 50-minute session.
Intake (Initial session) $220 per 90-minute session.
I offer a free 30 minute telephone consultation to discuss any questions you may have regarding the counseling process.
Payment is due at the time of services. Cash, Check, Money Order are acceptable as payment. Please make checks payable to Brandy Hofstad.

This includes Flexible Spending Accounts (FSA) and Health Savings Plan (HSA)
Insurance: I am not contracted with any insurance panels. However, many health insurances offer an "out-of-net-work" or "non-network" provider options. Insurance providers generally reimburse 30-80 percent of my fee. I will be happy to provide a receipt upon payment for each session, which you can submit to your insurance provider. I am also willing to submit claims directly on your behalf on a monthly basis. Please check with your insurance provider for additional information. Common questions to ask insurance providers are:
- Do I have mental health benefits?
- What is my deductible and has it been met?
- How much does my plan cover for an out-of-network provider?
- Is approval required from my primary care physician?
- What is the fax number or address to submit receipts?
Individual intake form for new clients (Please fill out and bring to your first session).
Additional forms will be discussed during the first session. These forms include: confidentiality, authorization and release of information (ROI), counselor disclosure, rights and responsibilities for youth under 18 years of age, and cancellation policy.
If you fail to cancel a scheduled appointment, I cannot use this time for another client and you will be charged for the entire cost of your missed appointment. A full session fee is charged for missed appointments (no show's) or late cancellations (less than a 24-hour notice). Exceptions may be made for emergencies, illness or inclement weather. If you fail to let me know in advance (more then 24 hours noice) that you will not be at your scheduled appointment, there will be a charge. You're responsible for letting me know if you're unable to attend a scheduled appointment due to; school breaks, family vacations, being out of town, etc. A bill will be mailed directly to all clients who do not show up for, or cancel an appointment.
